The cost of ketamine therapy usually has three parts: the medical evaluation, the medicine and dosing visits, and the therapy.

Insurance usually does not pay for ketamine prescribed off-label, while Spravato is often covered with prior authorization.

Therapy here, including the preparation and integration sessions of ketamine-assisted psychotherapy, can be billed to insurance with these plans: UnitedHealthcare, Aetna, Cigna, Blue Cross Blue Shield, Florida Blue, Oscar, Optum, Oxford and Carelon Behavioral Health. Only the ketamine dosing sessions are cash-pay.

The three parts

How much does ketamine therapy cost?

When people ask me what ketamine therapy costs, the honest answer is that it depends on what is included.

Most programs are built from the same three parts. They are just packaged and billed in different ways.

The medical evaluation

A licensed medical provider reviews your health history and current medications to decide whether ketamine is safe and appropriate for you.

The medicine and dosing visits

This is the ketamine itself and the time you spend taking it with support. Depending on the setting, that could be a clinic visit, an infusion, or a medicine session with a therapist.

The therapy

Some programs include no therapy at all. Others build in sessions before and after each dose to help you prepare and make sense of what comes up.

So two programs with a similar price can be very different in what you actually get. It helps to look at how the care is delivered before you compare totals.

How it is delivered

IV ketamine clinics, Spravato and ketamine-assisted psychotherapy

There are three common ways people receive ketamine for mental health. Each one is delivered and billed a little differently.

IV ketamine clinics

IV clinics give ketamine as an infusion, usually over a series of visits. A medical team monitors you during each one.

Most IV clinics bill you directly, and therapy is usually not part of the visit.

Spravato

Spravato is given at a certified clinic, and you stay there to be monitored after each dose.

Because it is FDA approved, it is billed more like a standard medical treatment and is often submitted to insurance.

Ketamine-assisted psychotherapy

A licensed medical provider evaluates you and prescribes ketamine off-label. A therapist provides the psychotherapy around the medicine sessions.

The medical part and the therapy part are usually billed separately, by different providers.

Where therapy comes in

Why therapist-led care is set up differently

In ketamine-assisted psychotherapy, the therapy is not an extra. It is part of the treatment, and it is part of the cost.

Before any medicine session, we meet for preparation. We talk about what is bringing you in, what you want to work on, and how to feel steady going in.

After each medicine session, we meet for integration. We look at what came up and how it connects to your daily life, your relationships and your patterns.

For many people, these sessions are where a lot of the lasting change happens. That is why the price of a single dose does not tell you much on its own.

If you want to see how the whole process works, I describe it step by step on my page about ketamine-assisted psychotherapy in Orlando.

Insurance

Is ketamine therapy covered by insurance?

Coverage depends on the type of care and on your plan. This is the general pattern.

Usually covered

  • Spravato, with prior authorization, at a certified clinic
  • Sometimes, the medical evaluation visit
  • Sometimes, psychotherapy sessions

Usually not covered

  • Ketamine itself, when prescribed off-label
  • IV ketamine infusions for mental health
  • Ketamine lozenges

At my practice, I accept these insurance plans for preparation and integration therapy sessions: UnitedHealthcare, Aetna, Cigna, Blue Cross Blue Shield, Florida Blue, Oscar, Optum, Oxford and Carelon Behavioral Health. Only the ketamine dosing sessions are cash-pay.

Every plan is different. Before you start, it is worth calling your insurance company and asking about each part separately.

Comparing coverage

Spravato vs. ketamine-assisted psychotherapy: coverage

Spravato is FDA approved for treatment-resistant depression. That is the main reason insurance plans often cover it, usually with prior authorization.

Ketamine-assisted psychotherapy uses ketamine prescribed off-label, so the medicine is usually not covered.

With me, the dosing sessions are cash-pay. The therapy sessions, meaning the preparation and integration work, can be billed to insurance.

Neither option is right for everyone. If depression is the main reason you are looking into this, my page on ketamine-assisted therapy for depression explains that work in more detail.

Medicaid

Does Medicaid cover ketamine therapy?

Medicaid rarely covers ketamine when it is prescribed off-label.

Coverage for Spravato depends on the state plan and its rules.

The most reliable way to know is to call your plan and ask about the specific treatment you are considering.

I want the cost to be clear before you decide anything.

I accept insurance for therapy with these plans: UnitedHealthcare, Aetna, Cigna, Blue Cross Blue Shield, Florida Blue, Oscar, Optum, Oxford and Carelon Behavioral Health. That covers regular therapy and the preparation and integration sessions of ketamine-assisted psychotherapy.

I do not prescribe ketamine. A licensed medical provider does the medical evaluation and prescribing, and the medicine and medical visits are billed separately by that provider.

Medicine sessions happen in person in Orlando, or at home in Florida with me on video the whole time, when the medical provider decides at-home dosing is safe. I see adults in person at my office and by secure video anywhere in the state, so ketamine-assisted therapy in Florida is available to you outside Orlando too.
